I understand that prior to being offered employment with Griffin Beverage I may be requested to take a pre-employment examination in which I may be required to submit to urinalysis, blood tests or other screening as part of the pre-employment process for the purpose of detecting drug and alcohol use.  Applicants tested will be required to sign a consent/release form prior to testing.  Any applicant refusing to sign the consent form or take a pre-employment physical will be disqualified from further consideration.

This pre-employment physical will be performed by a licensed, certified testing service and all applicants tested will be required to give a sample of the body fluid tested in the presence of an employee of the testing service.  A positive test may result in denial of employment.  In the event I have a disability in which will affect my ability to take the test I will so inform Griffin Beverage prior to the administration of the test so that a reasonable accommodation can be made.  Requested accommodations may include accessible testing sites, modified testing conditions and accessible testing formats.  Griffin Beverage reserves the right to require medical documentation concerning the need for the accommodation.”
